PC R 02-1839EXHIBIT "A" CONDITIONS OF APPROVAL CO�NDITIONAL USE PERMIT AMENDMENT NO. 02-006 ARROYO GRANDE COMMUNITY HOSPITAL, LOCATED AT 345 SOUTH HALCYON ROAD GENERAL CONDITIONS 1. The applicant shall ascertain and comply with all State, County and City requirements applicable to this project. 2. Development shall occur in substantial conformance with the plans on file at the Community Development Department office and marked Exhibit "A". COMMUNITY DEVELOPMENT DEPARTMENT 3. Prior to issuance of a Building Permit, a Cultural Resources Survey shall be submitted to the Community Development Department for the project site. 4. A qualified archaeologist, approved by the City, and a qualified Chumash representative shall be present to monitor all earthmoving activities on the project site. If a qualified Chumash representative cannot be made available within sixty (60) days of the project approval date, the project may move forward under the supervision of the qualified archaeologist alone. The archaeologist and/or Chumash representative shall maintain records of all pertinent observations and evaluations. 5. Proposed lighting shall not affect power for the Hospital. 6. All outdoor lighting associated with nonresidential uses shall be shielded and directed away from surrounding residential uses. Such lighting shall not exceed 0.5 foot-candles of illumination and shall be demonstrated to the Chief Building Inspector prior to Building Permit issuance. 7. All aisles, approach lanes, and maneuvering areas shall be clearly marked with directional arrows to simplify vehicular movement. 8. All parking facilities shall be graded and drained to dispose of surface water, subject to the approval of the City Engineer. Surfacing; curbing and drainage improvements shall be sufficient to prevent the free flow of water onto adjacent properties or public streets or alleys, and to avoid standing pools of water within the parking facility. 9. All automobile parking spaces shall be clearly outlined with painted lines on the surface of the parking facility. 10. All handicapped parking spaces shall be striped and marked according to applicable State standards. Each parking space reserved for the handicapped shall be identified by a permanently affixed reflecting sign, of a construction and material approved by the Building Official, displaying the international symbol of accessibility. The sign shall have a minimum size of 70 square inches, and shall be centered at the interior end of the parking space at a maximum height of 80 inches from the bottom of the sign to the finished grade of the parking space. 12. At the entrance to the parking facility a sign with a minimum size of 17 inches by 22 inches shall be posted, stating clearly and conspicuously in letters with a minimum height of one inch the following: "Unauthorized vehicles not displaying distinguishing placards or license plates issued for physically handicapped persons may be towed away at owner's expense." 13. Parking spaces reserved for the handicapped shall have a surface identification in blue paint of at least three square feet in size, duplicating the international symbol of accessibility. 14. After demolition and prior to commencement of construction, the landscape plan shall be submitted and approved by the Parks and Recreation Director 15. All landscaped areas required for parking facilities shall comply with the following standards: a) Planting areas shall be served by an adequate, water-conserving irrigation system. b) Deep root planters along Fair Oaks Road, and as approved by the Director of Parks Recreation and Facilities. c) All planted areas shall be continuously maintained in a healthy, growing condition, shall receive regular pruning, fertilizing, mowing, and trimming, and shall be kept free of weeds and debris by the owner or person in possession of such areas. Any damaged, dead, or decaying plant material shall be replaced within thirty (30) days from the date of damage. BUILDING AND FIRE DEPARTMENTS 16. Prior to relocation of parking lot lights, an Electrical Permit shall be obtained through the Building Department. 17. The project shall comply with the most recent editions of the California State Fire and Building Codes and the Uniform Building and Fire Codes as adopted by the City of Arroyo Grande. 18. All fire lanes must be posted and enforced, per Police Department and Fire Department guidelines. A sign shall be posted at all entrances that reads "All red curbs and driveways are designated as Fire Lanes. No Parking. Vehicles will be towed. CVC 22500.1 ". Exhibit A— Resolution No. 1839 Conditional Use Permit No. 02-006 Arroyo Grande Community Hospital Page 3 19. All parking to comply with A.D.A. & State Accessibility requirements. PARKS AND RECREATION DEPARTMENT 20. Prior to removal of any trees, a Tree Removal Permit shall be obtained from the Parks and Recreation Department. PUBLIC WORKS DEPARTMENT 21. Fees - The applicant shall pay all applicable City fees at the time they are due. 22. Site maintenance - The developer shall be responsible during construction for cleaning city streets, curbs, gutters and sidewalks of dirt tracked from the project site. The flushing of dirt or debris to storm drain or sanitary sewer facilities shall not be permitted. The cleaning shall be done after each day's work or as directed by the Director of Public Works or the Community Development Director. 23. Encroachment permits - The applicant shall be responsible for obtaining an encroachment permit for all work within a public right of way. 24. Grading Plan- Prior to the start of any grading operations, a grading, drainage, and erosion control plan shall be submitted to the Public Works Department, and approved. The design shall be subject to the approval of the Director of Public Works 25. � 27. � Grading - Curb, gutter and sidewalk - The applicant shall replace any damaged curb, gutter and sidewalk. Pavement Design - The main parking lot entrance path shall be designed to handle large trucks. (TI = 5.0). Maximum Cross Fall - The new parking lot shall be designed with a maximum cross- fall of 5%. Double Stripping - All new parking lot spaces shall be delineated with double striping. 29. 100 year Storm Design - All drainage facilities shall be designed to accommodate a 100 year storm flow. Any and all drainage designs are subject to the approval of the Director of Public Works. 30. Curb Returned Entrance - The driveway entrances shall be constructed with curb returns with a minimum radius of 15', and concrete cross-gutter and spandrels. Fossil Filter - All new on-site private drainage structures shall be equipped with a fossil filter and debris catcher. A regular maintenance and cleaning program for the on-site drainage facilities shall be implemented by the applicant. Both the filters and the maintenance program shall be subject to the approval of the Director of Public Works. : ■ .
